MINIMALLY THORACOSCOPIC AND LAPAROSCOPIC MCKEOWN ESOPHAGECTOMY: HOW I DO IT - STEP BY STEP

Introdução

Minimally invasive esophagectomy is the surgical treatment of choice for esophageal cancer. Videothoracoscopy and laparoscopy aim to reduce the rate of respiratory complications associated with thoracotomy, taking advantage of the benefits of reduced mortality from minimally invasive techniques. We brought a case of a female patient, 54y, with mid-esophageal squamous cell carcinoma after chemoradiotherapy (CROSS trial). There are several variations for the technique, but we bring a standardized way of performing a step-by-step minimally invasive esophagectomy to the McKeown procedure.

Objetivo

The purpose of this video is to present an easy step-by-step approach to a minimally invasive McKeown esophagectomy for squamous cell carcinoma and to address technical considerations of the procedure.

Casuística

Case report, female patient, 54 years old, with midesophageal squamous cell carcinoma after chemoradiotherapy (CROSS trial).

Resultados

The patient's postoperative course was well, with a low-output directed cervical fistula, and he was discharged on the seventh day with a diet with oral thickener. Anatomopathological result with complete pathological response (ypTypN0), continues in follow-up without evidence of disease.

Conclusões

Minimally invasive McKeown esophagectomy is a safe procedure when standardized and performed by trained professionals, with acceptable fistula rates and lower morbidity.
