Mucin-producing lesions of the lung - the chameleon tumor: Pictorial essay

Introdução

Presentations and findings of Mucin-producing lung tumors are variable. Note that more than one pattern may be present (overlapping is frequent). The tomographic appearance of these lesions is commonly multicentric (multi-lobar and bilateral).

Teaching points:


I. Identify the multiple radiological presentations (CT patterns) of invasive mucinous adenocarcinoma.
II. Recognize the patterns of spread over time of these tumors with emphasis on prognosis / outcome of cases.
III. Highlight important differential diagnosis of multifocal consolidative lesions.
IV. Discuss the initial evaluation and follow-up of patients with suspected pulmonary mucin-producing lesion.

1. The role of different techniques in the assessment of pulmonary mucin-producing lesion and optimal acquisition protocol:
a. Radiography;
b. Computed tomography (CT);
c. PET/CT;
d. Bronchoscopy images.

2. Spectrum of imaging findings - a CT and PET/CT-centered approach:
a. Consolidation;
b. Ground-glass areas;
c. Air-bronchogram;
d. Mucous bronchogram;
e. Centrilobular opacities and “tree-in-bud” appearance;
f. Nodules and masses;
g. Cystic spaces / Cavitations.

3. Patterns of spread and follow-up of cases:
a. Aerogenous / bronchogenic;
b. Hematogenous (rare);
c. Lymphatic (rare);
d. Pleural (rare);
e. Spread through airspaces (STAS) (not to be confused with a.).

4. Differential diagnosis to keep in mind:
a. Lymphoma;
b. Inflammatory myofibroblastic tumor (“pseudotumor”);
c. Amyloidosis;
d. Vasculitis;
e. Granulomatous diseases (tuberculosis, silicosis);
f. Hemorrhage;
g. Other alveolar infiltrates (proteinosis, eosinophilic diseases).
